1. You hereby grant your consent to SK K2 o.s., with registered office at Palkovice 158, Palkovice, ID No.: 22691600, file No.: L 7787, registered at the Regional Court in Ostrava (hereinafter referred to as the „Administrator“), to process the following personal data in accordance with Act No. 101/2000 Coll., on the Protection of Personal Data (hereinafter referred to as the „Personal Data Protection Act“):

• name and surname
• birth number
• e-mail
• telephone number

2. First name, surname, birth number, telephone number and e-mail must be processed for the purpose of registration, inclusion in the correct category and contact with the participant. This data will be processed by the Administrator for a period of 10 years.

3. You give your express consent to the above processing. Consent can be withdrawn at any time, for example by sending an email or letter to the contact details of SK K2.

4. Please note that under the Data Protection Act you have the right to:

• withdraw your consent at any time,
• request information from us about what personal data we process about you,
• request an explanation from us regarding the processing of your personal data,
• request access to this data and have it updated or corrected,
• request that we erase that personal data,
• contact us or the Data Protection Authority if you have any doubts about our compliance with our data processing
